Relay teams consist of four members plus two alternate members.  On the entry form, list the relay as an event for each member.  Indicate which team the member is on. "A" or "B", if applicable.  Event limitations apply to all team members:  Relay team composition must remain the same in the finals as in the semis.  Listed alternates may be used only in the case of sickness or injury.  No mixed age sex grouping allowed.  All athletes must be in same age group.

Please use a separate Declaration for each age division relay team.  Relay team members, including alternates, must be on the team Entry form.
